Brain of Cthulhu is a special boss npc only found in crimson worlds. You can summon this boss by breaking three pulsating hearts or by crafting a item called bloody spine. When the boss enters the world it will not take any damage until you kill enough of the creeper npcs. After that is completed the boss will be vulnerable to attack and can be knocked back with certain weapons.
